Clinical documentation is often approached as a task to complete rather than a critical component of ethical and effective care. This session explores advanced documentation practices that support clinical reasoning, continuity of care, and risk management. Participants will learn how to document with clarity and intention, navigate ambiguity and sensitive content, and write notes that both protect clients and support clinicians in delivering ethical, defensible care.
